FWPOA Application Form

To begin your membership in the professional organization representing Fort Worth Police Officers Association, Follow these three steps:

  1. Complete the application form.  (Link to fill out form to print.)
  2. Fill out the Payroll Deduct Form. (On the City Intranet, go to http://www.cfwnet.org/HR/forms.htm and look for the Payroll Deduction Form).
  3. Return Application with your signed Payroll Deduct Form to the POA Office when completed.
Your membership begins as soon as we receive this membership form. 
Your dues will be deducted from your paycheck the first pay period after your graduation.
